Extended Day Enrollment
(EDP)
If you plan to enroll your student in our EDP program, please fill out
this form and return it to school.

Band Information

If your student is moving from Intermediate 2 to 5th grade or is a new
University School student in grades 5, 6, 7, or 8, be sure to read the
information from our music teacher, Mrs. Call. All students in grades 5
through 8 are required to participate in band and will need an instrument.
